Workers’ Compensation
The Pennsylvania Bureau of Workers’ Compensation requires that certain information be provided to every employee at the time of hire and immediately after an injury occurs or as soon as reasonably possible following an injury. You can access the required incident report form and listing of approved providers in your area below.
